Preferred Dental Access Membership Program

Our practice has created this program to provide simple and affordable dental care for our patients without dental insurance. Our goal is to promote dental health and assist with the rising costs of dental care.
The Preferred Dental Access Program gives patients the opportunity to receive dental treatment at a reduced price, but it is not dental insurance. Here is how it works:

Included with Your Enrollment of $475 per year

Your enrollment entitles you to the following in a 12-month membership year:

• Two Routine Dental Examinations
• Two Dental Cleanings
• All Necessary X-Rays at the time of the routine dental examinations
• 20% Discount Off All Dental Restorative Procedures (excludes Velscope Cancer Screening, Oral Surgery, & Orthodontic Care)
• 10% Discount Off All Specialist Oral Surgery, Orthodontic and Periodontal care

Examples of Regular Fee / Discount Fee*

Please note that orthodontic case fees may vary based on patients’ needs and length of treatment. Listed fees are based on average treatment needs and treatment time.

• Payments are due at the time services are rendered.
• The initial annual fee is non-refundable.
• Additional family member: $350
• *Spouse and children up to age 23
• It is the patient’s responsibility to make and keep all appointments. Must provide 24 hour notice to cancel.
• Membership is non-transferable to another party. Qualifying members include immediate family members only.
• This program cannot be used in conjunction with any other dental insurance, discount plan or promotion.
• Our fees include no deductibles or exclusions due to pre-existing conditions.
• This program does not limit your dental care to yearly maximums, unlike most dental insurance programs.
